The Lost really went down in the rankings by the Isaac's Heart unlock. If he didn't unlock that, he would have been just above Tainted Bethany in S tier. Additionally, Zodiac and Dad's Lost Coin are fairly meh in terms of item unlocks. While those didn't lose him any points, they also didn't give him many.
